August 28, 2010: Healthcare reform is pushing the creation of entities called medical homes, accountable care organizations, and bundled payments. If you want to learn more about these organizations or how to establish one, here is a good website.
Accountable care organizations, electronic health records, economic pressures, and health reform is placing much more pressure on solo practices and small rural health clinics. So far the percentage of doctors practicing solo dropped from 41 percent in 1983 to 25 percent in 2008, and the portion of physicians in groups of six or less fell from 53 percent in 1997 to 42 percent in 2005. More and more young physicians, in particular, are going to work for hospital-owned groups. According to an oft-quoted study from the Center for Studying Health System Change, the percentage of physicians who are solo practitioners or are practicing in groups of fewer than 6 physicians fell from 53% in 1996 – 1997 to 42% in 2004 – 2005.

Rural health clinics were notably left out of the Medicare electronic health records stimulus payments and Senator Cantwell from Washington State has written a letter addressing this inequity. Here is the link.
